Adverbs like всегда, часто, иногда and никогда go before the verb: я никогда не хожу. Counted frequency uses раз, два раза plus в and the accusative: раз в неделю, два раза в месяц. Никогда always keeps a не with the verb.

- I never go to the gym.
- Мы встречаемся два раза в месяц.
- We meet twice a month.
- Она почти всегда завтракает дома.
- She almost always has breakfast at home.
The mistake to avoid
Two negatives are required: никогда не хожу, never just "никогда хожу".
